Procedures for Requesting Corrections to the WW2 Veterans List


PLEASE READ CAREFULLY BEFORE COMPLETING APPLICATION

The Veteran must meet the criteria set forth by the Monument Committee in order to appear on the finalized monument. The criteria that were prescribed are as follows:

1) Veteran of World War II - Dec 7, 1941 through Dec 31,1946
2) Veteran must have been discharged honorably (verifiable by military discharge/DD214)
3) Must meet and supply proof of Woburn residence : See Office of the City Clerk at     Woburn City Hall 10 Common St, Woburn, MA
4) Requests must be submitted in a timely fashion

All requests related to the World War II Honor Roll on Woburn Common must be made in writing, via a specific form. Each name change/addition requested, requires a separate form to be complete. Submitting a request form for inclusion only guarantees review by Monument Committee, and does not guarantee or imply that a particular Veterans name will appear on the finalized monument. Incomplete request forms will not be accepted. The form must be to be completed in its entirety & signed, and mail to
