Request for a Determination of Applicability (RDA)
For Applicants seeking a Determination of Applicabilty of the Woburn Wetlands Ordinance and /or the Massachusetts Wetlands Protection Act.

Notice of Intent (NOI)
For Applicants filing a Notice of Intent to do work in a resource area and/or buffer zone.

Abbreviated Notice of Resource Area Delineation (ANRAD)
For Applicants seeking confirmation of the boundaries of one or more resource areas.

Extension Permit
Applicants requesting an Extension Permit (to extend the date of the expiration of an active Order of Condtions) do not need a special form.  The Applicant shall submit a written request for an Extension Permit atleast 30 days prior to the expiration of the Order of Conditions.  The request must include the DEP file number, the date of expiration of the Order of Conditions, the period of time for which the extesion is sought (i.e., one additional year), the name, address and telephone number of the person making the request, and the reason the extension of time is needed in order to complete the project.  The request shall be accompanied by a $50.00 fee in the form of a check made payable to the City of Woburn. 

Certificate of Compliance
Upon completion of a project approved under an Order of Conditions, Applicants seeking a Certificate of Compliance shall submit seven copies plus an original of the Request for a Certificate of Compliance form along with the other items referenced in the final condition of the Order of Conditions to the Conservation Commission.
